  • Nebraska Application Updated 03-12-13. To expedite this process, please note the following:
    • Please print clearly and legible
    • On the form, under Authorization select both the Adult Protective Registry and the Child Protective Registry under Authorization
    • You do not need to have the form notarized (ignore the Self Check section)
    • All fields must be complete. Please put N/A in any fields that do not apply to you (i.e., no email address)
    • Please use street addresses - P.O. Box will not be accepted
    • Your hand-written signature is required on the 2nd page
