Plan for Meeting Outside of School Form
If you plan to meet your mentee outside of the school this summer you MUST complete a "Plan to Meet Outside of School" form. You are required to have a third party with you and your mentee at all times.Here is how to complete this process:
1. Download the form from:
http://www.teammates.org/chapters/metroomaha.htm
2. Complete the form (include the third party name and a detailed plan--where you are going, when, how long, etc.,)
3. Parent Signature It is vital that you communicate with the parents/guardians the details of your plan. Get the form to the parent/guardian to sign and set up a way to get the form back. This step can be hard to do, so make sure that you have good contact information for your mentee and their parents/guardian before school is out!
4. Turn in the form Once the form has been completed and signed by the parent/guardian of the mentee submit the form and your drivers license and insurance copies to the Metro Omaha TeamMates Office.
